Appointments can be scheduled at any of our convenient locations.  In addition, a physician is on-call 24 hours a day, 7 days a week for emergencies.

Other items required for appointment efficiency:

Bring your insurance card.  If you are being seen for an Automobile Accident or a Workmen’s Compensation injury, please bring the necessary documentation that contains the insurance carrier’s address, phone number, claim number and the date the accident occurred.

A referral is required from your Primary Care Physician for HMO patients and is necessary for appointment confirmation and reimbursement.

All Co-payments and deductibles are due at the time services are rendered.

Patients under 18 years of age must be accompanied by a parent or guardian or present a signed wavier.

Patients that cannot understand and converse in English should bring an interpreter.

APPOINTMENT CANCELLATION:  We ask that you provide a 24 hours notice so that we can provide this time slot for another patient who is in need of an earlier appointment. Your courtesy is appreciated.
